Welcome to Shoreline VIP Home Care LLC**. These Terms and Conditions govern the use of our home care services. By engaging our services, you agree to comply with the terms outlined below.
1. Services Provided Shoreline VIP Home Care LLC provides non-medical and specialized in senior care services, including but not limited to: - **Personal Care:** Assistance with bathing, dressing, grooming, toileting, and hygiene. - **Companionship:** Social engagement, emotional support, and recreational activities. - **Light Housekeeping & Meal Preparation:** Maintaining a clean environment and preparing nutritious meals. - **Medication Reminders:** Ensuring adherence to prescribed schedules. - **Mobility Assistance:** Support with walking, transfers, and fall prevention. - **Transportation & Errands:** Assistance with appointments, shopping, and outings. - **Specialized Care:** Support for individuals with dementia, post-surgery recovery, and chronic conditions.

2. Service Agreement- Prior to service commencement, a personalized care plan will be created and documented in a signed Service Agreement. This agreement will outline the specific services, schedule, and terms of care.

3. Client Responsibilities - Clients (or their authorized representatives) agree to: - Provide accurate medical and personal information to facilitate proper care. - Ensure a safe and respectful environment for caregivers. - Notify Shoreline VIP Home Care LLC of any changes in health status or care needs. - Provide necessary supplies for caregiving. - Allow caregivers appropriate access to the home for scheduled visits.

4. Payment & Billing - Payment terms will be specified in the Service Agreement. - Payments are due as per the agreed-upon schedule (e.g., weekly, bi-weekly, or monthly). - Late payments may be subject to additional fees or service suspension. - Any additional services outside the agreed plan will be billed separately.

5. Cancellation & Refund Policy - Clients must provide at least 48 hours of notice to cancel or modify services. - Last-minute cancellations may result in a fee. - Refund eligibility will be determined based on services used and contractual agreements.

6. Caregiver Assignment & Replacement - Caregivers are assigned based on client needs and availability. - If a client is dissatisfied with a caregiver, a request for replacement can be made. - Shoreline VIP Home Care LLC retains the right to reassign caregivers as necessary.

7. Liability & Limitations of Services - Shoreline VIP Home Care LLC provides **non-medica** services unless specified in the Service Agreement. - Caregivers do not administer medications, perform medical procedures, or diagnose conditions. - In case of a medical emergency, caregivers will assist in contacting emergency services but are not responsible for medical treatment. - Clients agree to release Shoreline VIP Home Care LLC from liability related to the inherent risks of in-home care.

8. Privacy & Confidentiality - We adhere to state and federal privacy laws, including HIPAA, to protect client information. - Personal and medical data will only be shared with authorized personnel or as required by law.

9. Compliance with Connecticut Laws - Shoreline VIP Home Care LLC operates in compliance with Connecticut state regulations for home care providers. Any disputes or concerns will be addressed following state guidelines.

10. Modifications to Terms - Shoreline VIP Home Care LLC reserves the right to update these terms at any time. Clients will be notified of any significant changes.
